It has slipped twice while driving under 30 mph. Fluid is full.

I’m still leaking fluid after replacing a leaking junction tube. The leak appears to be coming from below the thermostat housing which I heard will crack and leak but I have water coming out of the exhuast as well so I don’t think it’s that. I also had water boiling up in the spark plug wells (That may have occured when I tried to wash the rusty water off of the valve cover with a hose).
